Today is May 1, 2024. What the heck is going on in this country??? I received 2 charges from 2 different companies that are fraudulent charges. One was from WVI FRONT OFFICE TEMPE AZ. I don't do business with any Vascular Institute in Tempe AZ!!!! It has caused me some major stress bc, of course, today is the first of the month when I pay a lot of my bills ONLINE!!! The charge was $19.95 and finally as I am writing this I realized that yesterday I visited my eye doctor and purchased a required hot/cold eye mask for my eyes for $19.95 IN LAS VEGAS, NV!!! They offered it to me, and I thought why not? Faster t going to a drug store and probably the same price anyway. Now dealing with the second charge was the kicker!! It was a website that sold CBD products: gummies, drops, etc. AND all kinds of other products like women's pants, and tops and facial products, and it was a hodge-podge of a variety of products all on the one site. To say the least I have never been on that site before!! So now my dr card has been frozen with all the other companies I paid today included in the freeze. What does that mean? My bank is sending me a new dr card in 4-6 days. Now I must call every single company I paid bills through today and give them a different source of payment as they are ALL BEING DECLINED!!

I have no record and or charge on this date in 1/3/22. I Will not pay this charge unless I have received proof of service.

My credit card was charged $128 bythe company above. Who is this company and what am I being carged for? I have retina damage and cannot some many of your photos clearly. My name is Karyl Tomizuka at 520-327-8806

Unknown charge from above Display name. No idea how they got my credit card number. Date of transaction was for eye appointment. SW Eye Clinic Scottsdale refused to take my eye insurance plan and run charges under Medicare and then also run same visit under my eye insurance plan. Now they are making another charge. Did double billing and now making a charge for a co-payment which I don't have.
